CINA is proud to announce the release of the long awaited reporting module, Care Management Plus™.  A tool for population and patient management, Care Management Plus™ provides practices with a user friendly tool for mining data from the practice EMR, thus creating the reporting capabilities required to effectively managing care in this era of increased accountability. 

 

Coupled with the CINA Protocol Engine™, Care Management Plus™ provides the infrastructure necessary for management and benchmarking across a practice or community, and for meeting many of the requirements for Meaningful Use.

The CINA Reporting Module takes Registry functionality to the next level, providing a wide range of pre-programmed and custom reports aimed at giving the practice tools to address needs of the entire population—from management of large groups for preventive screening, down to the highest risk patients with chronic disease in need of intensive care management.

 

Highlights of Care Management Plus:

 

  • High level snapshot of commonly reported metrics for chronic disease management and prevention
  • Drill down capabilities at the organization, practice, specialty / department, provider or patient level
  • Identifies patients that are overdue or nearly overdue for specific tests, procedures, etc.
  • Supports custom reporting sets, making it easy to track metrics needed for specific programs, such as PQRI, BTE, IPIP, P4P, Medical home, etc.
  • Meets many of the criteria required for Meaningful Use
  • Allows for the creation of both graphs and charts to effectively communicate results
  • Data exportable to MS Excel or MS Access for mail merge, outbound contact tools, etc

 

CINA’s reporting module has given practices a new view into their clinical data.  Providers and administrators can now manage and track metrics by disease condition, prevention, or a defined set of measures.  Patient level data then provides the “back up” information needed to reach out to patients and providers for improvement activities.
