The NCQA seal is a widely recognized symbol of quality. For consumers and employers, the seal is a reliable indicator that an organization is well-managed and delivers high quality care and service. To be come recognized by the NCQA as a PCMH provider, providers are responsible for nine Physician Practice Connections standards, including 10 must-pass elements, which can result in one of three levels of recognition. Building on the joint principles developed by the primary care specialty societies, the PPC®-PCMH standards emphasize the use of systematic, patient-centered, coordinated care management processes.
