The true value of the electronic exchange of patient information can only be realized when the data held by community physicians and other providers is accessible and standardized in a way that enables the creation and deployment of population-based care tools across healthcare settings. This requires the establishment of baseline quality indicators, population-based disease registries and evidence-based care guidelines constructed from nationally recognized best practices.
 
Using the CINA tools for data aggregation and standardization, health information exchange (HIE) initiatives find an efficient, affordable way to facilitate active participation across the community. Best of all, there is no need for providers to change how they gather data at the point of care or to provide outside organizations with unfettered access into their internal systems.
 
Along with technology, CINA brings a wide breadth of experience in data measurement as well as the Patient Centered Medical Home to HIE’s. Serving as a data aggregator for several national initiatives—Bridges to Excellence (BTE), PQRI (CMS), Improving Performance in Practice (IPIP)—CINA understands the many nuances of reporting quality measures, especially when reporting spans multiple provider organizations and electronic systems. 
 
Finally, the CINA Medical Home application provides a mechanism to enhance the Medical Home for provider groups with an EMR. CINA also provides a “place to start” for those groups that have not yet implemented an EMR.
